To schedule an appointment with Dr. Chitiva, please complete the Contact Form to determine if
Dr. Chitiva is the right fit for your needs.

Once scheduled, new patients will receive an intake form and a service agreement. Completion of these forms is required prior to the initial consultation.

Currently, Dr. Chitiva is IN-NETWORK with the following insurance providers:

Aetna, Cigna, United Healthcare, Florida Blue/BCBS
Additionally, Dr. Chitiva accepts cash payments and patients using out-of-network health benefits.
